Don't know what it means? I don't have time to explain it, but some people feel it's homophobic, while others think it's a harmless joke.
I lean to the homophobic argument, and now it seems Juelz Santana agrees.
On the Juelz song, "Mic Check" Santana clearly says:

Now, if you're one of the dozen who picked up Santana's album yesterday, you may have noticed that he takes out the "No Homo" part of this lyric. And yes, this is the dirty version of the album.
Before I get into what I think about this, you gotta find it funny that Juelz would change a lyric where he's bragging that whatever he says he means it! I guess "No Homo" is the exception.
If Juelz made this change because he's growing older and becoming more enlightened, then let's clap it up for this brother. However, if he's just afraid of the backlash that homophobic lyrics may receive, then I can't respect dude's politically correct move. That ain't rap music!
Who will make a fuss about the No Homo lyric anyway? Even some Hip-Hop journalists are homophobes themselves, willing to co-sign the most slur filled speech, as long as the writer is speaking metaphorically.
